This site is hosted by WordPress.com, which may use cookies and collect technical information about site visits. This website does not collect clinical intake forms. If you email me, I receive the information you send; please avoid including sensitive health information in regular email.

Appointments and records

My independent practice uses Jane for appointments, intake forms, and clinical records. Appointments booked through Novelty Physiotherapy and Wellness Clinic use its clinic-managed Jane account and associated privacy procedures. You can read Jane’s Privacy Notice.

How information is used and protected

I use personal information to respond to inquiries, provide psychotherapy, manage appointments and payments, maintain records, and meet legal or professional obligations. Information is shared with consent or as permitted or required by law. Reasonable safeguards are used to protect records, which are retained according to applicable requirements.

Access, corrections, and concerns

To request access to or correction of your records, ask a privacy question, or raise a concern about my independent practice, contact hedyanvari@gmail.com. For records held by Novelty, I can direct you to the appropriate clinic contact. You may also complain to the Office of the Information and Privacy Commissioner for BC or the Information and Privacy Commissioner of Ontario, as applicable.
